The Forever 35 Facelift is a trending phrase in the world of plastic surgery today. The goal is clear: patients interested in this type of procedure want undetectable enhancements that age gracefully. Oftentimes, surgeons purport to achieve this look with a ‘deep-plane’ facelift—another trending phrase, but one that often gets misused. It is meant to refer to a type of technique that repositions the deeper layers of the face to achieve transformative yet natural-looking results (1); however, many surgeons who claim to perform deep-plane lifts actually still use outdated, inadequate techniques that leave patients looking overdone.
Board-certified plastic surgeon Dr. Rian A. Maercks of The Maercks Institute is an aesthetic, craniofacial, and reconstructive plastic surgeon licensed in both Florida and New York. He is known as a pioneer of many advanced techniques, including the MAERCKS Lift™, which provides patients with that effortless, forever-35 look they are striving toward.

Before and After Photos
About the MAERCKS Lift™ and Forever 35 Rejuvenation
Traditional facelifts typically only address the superficial layers of the skin and the superficial musculoaponeurotic system (SMAS). Dr. Maercks’ MAERCKS Lift™ goes beyond this with KEDGE® Suspension, a proprietary system that repositions the mid-face and neck as a single, natural composite. The procedure is performed wide awake under local anesthesia, with patients often conversing and even singing during surgery. There’s zero general anesthesia, zero visible scars, and virtually zero social downtime.
Dr. Maercks notes that while many practices have started labeling their procedures as “Forever-35” or “deep-plane,” few perform genuine deep-plane dissection, while even fewer support the ligamentous origins of the face.
“True rejuvenation is about restoring deep support, not stretching the skin.”
– Dr. Maercks
Candidates
“The new generation of facelift patients want to look like themselves, not ‘done.’ They want to look how they feel inside. The Forever-35 look isn’t about turning back the clock; it’s about dissolving the idea of time altogether.”
– Dr. Maercks
Suitable candidates for a Forever 35 Facelift include those interested in addressing signs of aging around their mid-face, lower face, and neck, all while maintaining utmost discretion. Roughly half of the patients who undergo this procedure are between the ages of 32 and 42, but age is not necessarily a determining factor. This demographic is drawn to the Maercks Institute’s combination of artistry, safety, and subtlety.
It is important that patients are in good general health without any underlying conditions that could affect their ability to undergo the procedure. Patients should ideally be nonsmokers or willing to quit nicotine entirely. Dr. Maercks will finalize each patient’s candidacy for a facelift during their consultation.

Preparation
Dr. Maercks will provide you with a set of detailed pre-operative instructions to help you get ready for your Forever 35 Facelift. While instructions may vary, they often include guidelines such as:
- Refrain from taking certain blood-thinning medications and supplements.
- Arrange transportation to and from the procedure.
- Prepare to take time away from work and other commitments.
- Stop smoking/nicotine use several weeks in advance until at least a month after your appointment. (2)
- Avoid drinking any alcohol in the days before and after your procedure.
- Complete any requested lab tests in advance.
Procedure
“‘Forever-35’ is not a procedure. It’s a promise; an aesthetic ideal rooted in balance, subtlety, and deep structural support. We’ve been achieving that for over a decade through the MAERCKS Lift™, which works in the deep plane to reposition, not pull, while keeping patients completely awake and pain-free.”
– Dr. Maercks
The Maercks Institute now performs two to three awake deep-plane facelifts daily. When you arrive for your appointment, you will be greeted by a member of our team and led to your private treatment suite. Dr. Maercks will cleanse your skin and administer local anesthesia, meaning you will remain awake and comfortable throughout your procedure, able to talk—or even sing—freely.
Dr. Maercks will then reposition your mid-face and neck tissues using KEDGE® Suspension, a proprietary system he developed that repositions these regions as a single unit. This is the secret to the subtlety and longevity of Forever 35 Facelift results—Dr. Maercks’ techniques establish long-lasting stability and an exceptional natural look.
Recovery
Dr. Maercks performs the Forever 35 Facelift on an outpatient basis, which means you will be able to go home on the same day as your appointment. Thanks to Dr. Maercks’ cutting-edge techniques, there is essentially zero downtime associated with this procedure. This is a significant advantage over a traditional facelift, which typically requires patients to take at least two weeks off work to rest. After a Forever 35 Facelift with Dr. Maercks, you will feel ready to return to your normal routine in no time.
Results
You will be able to enjoy the results of your Forever 35 Facelift immediately after your procedure. You will see more defined cheekbones, a sculpted chin and jawline, tighter-looking skin, and a more youthful appearance overall. It is important to note that your results will still look and feel like you, just naturally refreshed!
One viral case from our practice describes a 35-year-old patient who underwent a wide-awake MAERCKS Lift™: She returned home from her procedure the same day, and kept the transformation secret from her partner! The only thing that was different about her—other than her stunning results—was that she “wore a headband for a few days.”
The results of this procedure are incredibly long-lasting, and unlike other techniques, will not result in unnatural warping over time. Patients often see their results for a decade or longer.

FAQ
What concerns can a Forever 35 Facelift address?
Dr. Maercks can perform a Forever 35 Facelift to address signs of aging in the middle and lower portions of your face, as well as your neck. Common concerns this procedure can improve include creases, wrinkles, jowling, sagging, loss of definition, and more.
What type of anesthesia is used during a Forever 35 Facelift?
Dr. Maercks can perform this procedure using local anesthesia. This is something that not every surgeon offers. It allows patients to be awake and comfortable throughout their procedure. Avoiding the use of general anesthesia also makes the post-operative period much smoother, as you will not have to deal with the associated side effects.
Will there be visible scarring after this procedure?
No! Dr. Maercks’ advanced closure techniques and incision placement allow for a completely natural final look. Any scars will be very discreet, and no one will be able to tell that you had any procedure done. They will think you look great, but will not be able to pinpoint exactly why!
What is the difference between a deep-plane facelift and a Forever 35 Facelift?
The Forever 35 Facelift is technically not a standardized surgical procedure; it is an aesthetic ideal focused on subtle, undetectable rejuvenation that restores deep structural support rather than pulling the skin. While many surgeons use the term “deep-plane facelift,” it is often misapplied. True deep-plane rejuvenation requires genuine mid-facial release and repositioning of deeper tissues, not superficial tightening. Dr. Maercks can achieve the forever-35 look with his MAERCKS Lift™. What makes Dr. Maercks’ approach to the Forever 35 Facelift different? At The Maercks Institute, the Forever 35 Facelift is achieved through Dr. Maercks’ proprietary MAERCKS Lift™, which works in the deep plane to reposition the mid-face and neck as a single, natural composite using KEDGE® Suspension. Performed wide awake under local anesthesia, this approach prioritizes balance, longevity, and authenticity, so patients look like themselves, just effortlessly refreshed!
Can my Forever 35 Facelift be combined with another procedure?
Yes, Dr. Maercks can incorporate another procedure into your treatment plan to complement the appearance of your results. These often include other facial rejuvenation procedures, surgical or nonsurgical. If you are interested in combining your procedure, we recommend scheduling a consultation.
How long do the results of a Forever 35 Facelift last?
Results from a Forever 35 Facelift with Dr. Maercks are designed to be exceptionally long-lasting because the procedure restores deep structural support rather than simply tightening skin. By repositioning the mid-face and neck as a single, stable unit using the MAERCKS Lift™ and KEDGE® Suspension, patients often enjoy natural-looking results for a decade or longer. Importantly, the face continues to age gracefully over time without the distorted or “pulled” appearance that can develop with more superficial techniques.
