Brow & Eyelid Lift
The eyes play a central role in how we express ourselves. When the brows descend or the eyelids develop excess skin, the upper face can appear tired, heavy or older than it feels.
Brow and Eyelid Lift surgery focuses on restoring balance to the upper face while preserving natural expression and individual identity.
Dr Rian A. Maercks takes an anatomy-led approach, assessing the position of the brows, the structure of the eyelids and the relationship between the two. Rather than automatically treating each area separately, the objective is to determine what is contributing to the appearance of heaviness and develop a treatment plan accordingly.
The philosophy is simple: refinement, not overcorrection.

Understanding The Brow & Eyelid Surgery
The brow and eyelids work together to shape the appearance of the eyes and upper face. Changes in either area can affect how open, rested or expressive the eyes appear.
A brow lift addresses the position of the eyebrows and may soften certain forehead lines, while eyelid surgery, also known as blepharoplasty, addresses excess skin and selected underlying tissue around the upper and/or lower eyelids.
For some patients, the concern originates primarily from the brow. For others, excess upper eyelid skin is the main factor. In selected cases, both areas may contribute to the appearance of heaviness.
A comprehensive assessment helps determine which procedure, or combination of procedures, is appropriate.
THE BROW LIFT
Restoring the Position of the Brow.
A brow lift, also known as a forehead lift, is designed to elevate the eyebrows where descent contributes to a heavy or tired appearance.
As the brow changes position with age, it can contribute to:
-
A heavy or lowered brow.
-
A tired, sad or stern-looking expression.
-
Deep forehead creases or frown lines.
-
Increased hooding around the upper eyelids.
The aim is to restore a more balanced brow position while maintaining natural expression. The appropriate technique depends on brow anatomy, hairline, forehead proportions and individual goals.
THE EYELID LIFT
Refining the Eye Area
Eyelid surgery, or blepharoplasty, addresses changes affecting the upper and/or lower eyelids.
Depending on the anatomy, this may involve carefully refining excess skin, repositioning or adjusting underlying tissue, and improving the contour around the eyes.
Eyelid surgery may be considered for concerns such as:
-
Loose or creased upper eyelid skin.
-
Hooding that obscures the natural eyelid crease.
-
Under-eye bags or puffiness.
-
A heavy or tired appearance around the eyes.
The focus is on achieving a more refreshed appearance while preserving natural eyelid shape, function and facial expression. Excessive tissue removal should be avoided.
UPPER OR LOWER EYELID SURGERY?
Some patients may benefit from treatment of the upper eyelids, the lower eyelids or both. A combined approach is not necessary for everyone.

A PERSONALISED SURGICAL APPROACH
Every brow and eyelid has different anatomical characteristics. Treatment is therefore tailored to the individual rather than based on a standard technique.
Your assessment may consider:
-
Brow position: The height, shape and position of the eyebrows.
-
Eyelid anatomy: Skin excess, tissue distribution and eyelid structure.
-
Forehead and hairline: Their relationship to brow position and surgical planning.
-
Eye shape and expression: How the proposed changes may affect your natural appearance.
-
Facial balance: How the upper face relates to the cheeks and surrounding features.
Depending on your needs, treatment may involve a brow lift, upper or lower blepharoplasty, or a combination of procedures.
Additional procedures are considered only when appropriate to your anatomy and goals.
WHY THE BROW AND EYELIDS MUST BE ASSESSED TOGETHER
One of the most important considerations in upper-face rejuvenation is understanding where the heaviness originates.
A lowered brow can contribute to excess skin appearing over the upper eyelid. Treating the eyelid alone may not address the underlying brow position. Conversely, a patient whose brow position is appropriate may benefit from eyelid surgery without a brow lift.
The distinction matters because the right procedure depends on the cause of the concern, not simply how it appears in the mirror.
Dr Rian assesses the brow, forehead and eyelids together to determine the most appropriate approach for each patient.
The objective is not simply to lift the brow or remove skin. It is to restore a more open, balanced and natural-looking upper face.
